Despite coming out the other side okay, The Patient is still feeling worn out from the battle. Whether the other side is the afterlife or surviving the illness, recovery time will still undoubtedly be needed.

Lyric: I am not afraid to keep on living
 I am not afraid to walk this world alone
Annotation:
In this line, The Patient is saying that although cancer has taken over most of his life, he is not afraid to start over and go on as if everything is fine, even if nobody is with him. (It can also be interpreted as Gerard Way trying to convince his brother Mikey to not take his own life.)

PROPOSED SUGGESTION: I could be mistaken because I heard this a while ago, but this is about Mikey because he struggled with depression. Most of this song is about Mikey, so I’m pretty sure this line is Gerard talking to Mikey and encouraging him to stay alive.

Gerard may mean that his “famous last world” were that he is not afraid to keep playing and travel to tours with the band but without his brother mikey, but actually, he is terrified.

Lyric: I am not afraid to keep on living
Annotation:
On the studio album, during this chorus the guitars and drums fade away to be replaced with an organ for the last words of the song, and in the music video they continue to the last word, insinuating The Patient’s journey to either heaven or hell.

 Awake and unafraid
 Asleep or dead

Gerard says, “I see you lying next to me” meaning his love came to him in the hospital on his death bed. “With words I thought I’d never speak” meaning he’s telling her how much he loves her before he passes away. The reason for him to say “thought I’d never say” is because he had so much hate for her leaving him but he feels over whelmed to tell her how much he loves her. The next part in these lyrics is confusing because they contradict each other. But he says “awake and unafraid”. but this doesn’t mean he is literally awake this means that he is of himself or that he is aware of his surroundings. “Unafraid” saying he loves her with no fear of what might happen to him. The next line “asleep or dead” meaning is he literally asleep? or would he be dreaming of his love coming to him at his death bed? “Dead” meaning or is he really in the next world and her being there with him is a form of torture? Because a vision of her being there would be a way to mock him. He would not be able to truly tell her how he feels about her. These lyrics are hard to understand so this is just my opinion on what they mean.

This could count has two things. The Patient is “the savior of the broken, the beaten and the dammed.” The Love in this sense is him being the champion when he wants to live and escape, seen has the b side songs. The Second is Gerard Way himself the fan base is so crazy in a good way this lyric could be her reminiscing about the May Death Never Stop You tour he was drinking like crazy was doing drugs and was considering suicide.

Lyric: Is it hard understanding
 I’m incomplete?

The patient is coming to terms with the fact that he did not get to live the full life he always dreamed of. At the same time Gerard feels incomplete without his brother Mikey alongside him.

Lyric: Awake and unafraid (How can I see, I see you lying)

Annotation:
The Patient has gone through the entirety of his road to the end. He has resolved every riddle in his mind and forgiven himself for everything; he no longer has anything that attatches him to this world. There’s only life and death, and if he can close his life it’s because he no longer fears death. But that does not mean necessarily that he is now just waiting for death. He could just fall asleep  and wake up next morning to continue his life if necessary, because he no longer fears life.

Lyric: Honey, if you stay, you’ll be forgiven
Annotation:
The focus of this album’s story, The Patient, has accepted his fate. After all his struggles throughout the album, we reach the end of his journey – whether he dies or not is up to the listener’s own interpretation, but either way, he is going ‘home’ and nobody can change that. He is no longer afraid of what’s going to come next for him. As well as tying in with this album’s concept, this song (as already mentioned,) serves as a message to the band’s bassist – and the singer’s brother – Mikey Way. The band had been going through a particularly rough period while recording this album, and this culminated in Mikey having to take some time off to clear his mind. In  Issue 218 of Rock Sound Magazine, guitarist Frank Iero is quoted as saying this of the song: It gave us a new life, a new hope […] It really put us back in the mindset of making a great record. It was this song that brought the band back together and lifted their spirits, in order to finish the album, clearly making it an important track for everyone involved.

This line could refer to several things: Mikey’s POV that nothing Gerard can say will make him stay in the band Gerard’s POV that nothing Mikey can say will stop him from being with his brother The Patient saying that nothing can stop him from going home, whether Heaven, Hell or his physical home, as he succumbs to the disease.

This line could maybe be interpreted in Mikey’s POV (the song, after all, was written for him) as him asking Gerard if he would just understand Mikey’s leaving and not try to stop him.

His love for performing and music takes a toll but he can’t complain for being so successful and doing what he loves

The writer (Gerard) knows he made a mistake and cannot go back to fix it. Also if put into context of the album’s story, the patient (aka the album’s narrator) knows he can’t say anything to his loved one that will change the fact that he is ill and dying.

Lyric: F a jet black hotel mirror
Annotation:
when mirrors get old, dark spots appear on them. When a hotel has a mirror like this, it means that the hotel is bad. So he says that he’s on the other side away from his faulted cheap life.

Lyric: Can you see? My eyes are shining bright
 ’Cause I’m out here on the other side
Annotation:
The Patient is telling his lover that he’s already moved on and happy with his new life or happy in his afterlife. It could also be Gerard telling Mikey that he’s survived. He’s fought his depression, and he’s won, and he wants Mikey to know that it’s possible that he can fight it as well.

Lyric: So many bright lights to cast a shadow
 But can I speak?
Annotation:
No matter how good something is, there’s always a dark or negative side to it. The fame of being in My Chemical Romance may be nice, but it’s mostly directed towards Gerard, so the people can’t see Mikey and how talented he is.  In the context of the album, this could also be the bright lights of operation rooms and the shadows on the faces of the ill since all of the operations wear patients out.

Lyric: Now, I know that I can’t make you stay
 But where’s your heart?
Annotation:
This is Gerard reaching out to his brother Mikey, who left the band temporarily during  this album’s production. Gerard wrote this song for him. Another explanation is that this alludes to the characters in the album’s story. A guy’s lover leaves him, with her reason being his disease.

Lyric: A life that’s so demanding

Annotation:
Gerard’s life is crazy and chaotic, and people expect a lot from him. After all, he is the frontman of one of the  most influential bands of the 2000’s. This can be overwhelming, and when it gets to a certain point it breaks him down.  In terms relating to Mikey, it could mean that he wasn’t able to see Mikey’s weaknesses because he could barley deal with his own.

Lyric: Well, is it hard understanding
 I’m incomplete?
Annotation:
There is something missing in his life, but no one sees the emptiness but him.  Without Mikey, Gerard is incomplete (he is asking Mikey to stay)

Lyric: These bright lights have always blinded me
 These bright lights have always blinded me, I say
Annotation:
He is most likely talking about the Light that you see when you die, but it could also mean the hospital lights where he most likely died during Welcome To the Black Parade, making a parallel between the hospital and death. He also means (away from the story and in his real life) that fame had always blinded him from what was really going on.
